The Match Is Getting Harder — And So Is Residency

The numbers around the anesthesiology residency Match tell two stories at once, and they're in tension with each other. According to reporting compiled by Becker's ASC, more than 3,000 students applied to the 1,805 anesthesiology residency programs available in the 2025 Match — and roughly 40% of applicants went unmatched. A closely related figure from 2024 reporting put the unmatched rate at around 44% that year.

On one hand, that reflects genuine, growing interest in the specialty — anesthesiology is not a training pathway struggling to attract candidates. On the other hand, it means residency positions have not scaled anywhere close to the pace of applicant interest, creating a training bottleneck precisely at the moment the broader workforce data (see our companion article on the anesthesiologist shortage) shows the specialty needs more new attendings, not fewer.

The second-order effect on the residents who do match

That bottleneck has a second-order effect worth naming directly: it concentrates enormous pressure on the residents who do match — both to perform, given how competitive their spot was to secure, and often to absorb workload gaps left by the same broader staffing shortages affecting attending anesthesiologists. Burnout research on anesthesiology residents (documented in our companion article on resident burnout prevalence) already shows the training years carry real, measurable psychological strain — a strain that a widening applicant-to-slot ratio is unlikely to ease on its own.

None of this is something an individual training program can solve through GME policy alone. But it does make the case for programs to be deliberate about the day-to-day culture residents experience once they're in the building — because the pipeline pressure upstream isn't going away soon.
